Confidentiality

Everyone working in the NHS has a legal duty to maintain the highest level of confidentiality about patient information. Your medical history is recorded on our computer system and in paper records which are accessed by authorised users only.

Suggestions & Complaints

COMPLAINTS

We always try to give you the best service possible, but there may be times when you feel that this has not happened. If this is the case please ask for a leaflet at reception. If you wish to make a complaint, please telephone or write to the Practice Manager, Mrs Julie Finch, who will take full details of your complaint.

SUGGESTIONS

If you have any suggestions as to how we might improve our services, please use the suggestion box in reception or speak to your doctor or to the Practice Manager. 

Rights & Responsibilities

The care of your health is a partnership between yourself and the Primary Health Care Team. The success of that partnership depends on an understanding of each other’s needs and co-operation between us.

Our Responsibility to You

You have a right to:

• Be treated with courtesy and respect by every member of the Primary Health Care Team.
• Confidentiality.
• Receive health care on the basis of clinical need.
• Have any proposed treatment including any risks involved in the treatment and any alternatives explained to you before you decide whether to agree to it.
• Be prescribed appropriate drugs and medicines.
• Be referred to a consultant acceptable to you when your doctor thinks it is necessary.
• Choose whether or not you want to take part in medical student training.
• Have access to see your medical records subject to the limitations of the law.

Your Responsibility to Us

• Please treat NHS staff, fellow patients, carers and visitors politely, and with respect. We will not accept violence, racial, sexual or verbal harassment.
• Do what you can to look after your own health, and follow advice on a healthy lifestyle.
• Care for yourself when appropriate. (For example, you can treat yourself at home for common ailments such as coughs, colds and sore throats.)
• Listen carefully to advice on your treatment and medication. Tell the doctor about any treatments you are already taking.
• Keep your appointment or let us know as soon as possible if you cannot make it.
• Order your repeat prescription as per the instructions on this website. Please attend for review, when asked, before your next prescription is due.
• Inform us of any change of name, address or telephone number so our records are accurate. 
• Do not ask for information about anyone other than yourself.

Violence Statement

We will not tolerate violent or abusive behaviour towards any member of the Primary Health Care Team. The practice policy is to remove you from the list. In certain circumstances you may be given a warning.
