If you think that you may require any vaccinations relating to foreign travel you need to complete a travel questionnaire in the first instance – ideally at least 8 weeks before you travel. You can download the Travel Risk Assessment Form and email it to bridge.surgery@nhs.net or you can print a copy off and post it to the surgery/hand it to the receptionist.
Our travel nurse will then look at your form to determine what vaccinations you require (if any). You will then be contacted by a receptionist in order to make your appointment.
Generally travel vaccination appointments are on a Thursday afternoon.

Please note that from 1st August 2019, we will no longer be offering private vaccinations for travel purposes. These vaccinations, if required, can be provided by local pharmacies.
We will continue to offer the vaccines available on the NHS for travel purposes.
